# FunSurveys submission checklist — version 1.0

Use one kit per provider and country. These are blank templates, not evidence that any test has taken place.

## Diary
- [ ] Confirm the agreed assignment, country, dates, timezone and device.
- [ ] Record whether the account is new or existing and any waitlist or verification limit.
- [ ] Complete each observed day. Use 0 only after actually checking; leave unobserved values blank and explain missed checks.
- [ ] Give every started survey one current outcome: completed, screened out, technical failure, other exit or still pending. Explain later changes rather than double-counting.
- [ ] Include setup, screening, survey and administration time.
- [ ] Keep approved rewards, pending rewards and received money separate.
- [ ] Do not include the FunSurveys contributor fee in provider earnings.

## Evidence
- [ ] Check provider capture/confidentiality rules. Record a limit if capture is prohibited.
- [ ] Use only your own genuine, allowed captures; never generate dashboards or receipt evidence with AI.
- [ ] Give each file a neutral evidence ID and connect it to the diary/session and capture time.
- [ ] Submit flattened redacted copies, not raw identity/financial documents or sensitive originals.
- [ ] Remove names, emails, photos, IDs, bank details, security codes, account/transaction identifiers, private links and confidential study material.
- [ ] Inspect full-size exports and remove personal metadata. Preserve the relevant amounts, dates and outcomes.
- [ ] Record permission for each public image and agreed public credit separately.
- [ ] Record separate permission before any video, voice or social reuse.

## Cash-out
- [ ] Record method, displayed minimum, currency and fees.
- [ ] Use the status not_attempted, requested_pending, received, rejected or cancelled.
- [ ] Enter amount_received and received_at only for an evidenced actual receipt.
- [ ] Record why a withdrawal was not attempted. Do not pay or extend testing just to force one.
- [ ] Agree any later follow-up rather than silently extending the assignment.

## Report and delivery
- [ ] Summarize signup, observed availability, screen-outs, rewards, cash-out and limitations.
- [ ] Match every supported statement to an evidence ID or a dated official source.
- [ ] Explain gaps and contradictory records.
- [ ] Do not claim typical earnings, national averages or provider-wide payment reliability from one account.
- [ ] Submit only via the route agreed with the editor. The initial application mailbox is not an approved raw-evidence upload route.
